Assists the physician in his evaluation of patients, including the application of muscle, nerve, joint, and functional ability tests.

Treats patients to relieve pain and develop or restore function, using physical means such as exercise.

Directs and aids patients in active and passive exercises, muscle re-education, fine motor skill functional training, and activities of daily living, including prosthetic training.

Observes, records, and reports to the physician and the Director of Nursing the patient's reaction to treatment and any changes in patient condition without delay.

Instructs a patient in the care and use of medical equipment, prosthetic and orthotic devices.

Instructs health care personnel and family members in certain phases of therapy, when appropriate.  May give input into the evaluation of the care provided by Home Health Aides/ Certified Nursing Assistants.

Participates in interdisciplinary staff meetings when time permits, at least monthly, and reads and signs the written case conference meeting notes.  Also will provide, upon request, at least one in-service per year concerning his/her therapy practice to other employees of the AGENCY.

Documents all communication with other agency workers and physician on his/her notes or on conference notes.  Gives adequate notice when terminating service to the patient.

Valid state driver’s license and reliable automobile, current automobile insurance and be willing to operate personal car necessitated by nature of job. Must also maintain a good driving record.

Providing Agency with a copy of current CPR card, current professional license, and proof of TB testing, or signed waiver. Therapist is responsible for keeping current with these documents and providing Agency with copies of the same.

Good verbal and written communication skills with ability to read and communicate effectively in English.  Additional languages preferred, but not required.

In-take Nurse/Clinical Coordinator

In-take Nurse/Clinical Coordinator


Main functions: 

Screening and tracking of all home health/hospice patients, working as the liaison between the case managers, physicians and other clinical staff in coordinating the care of patients via phone and the follow-up of discharged patients from home care

Duties:

·       Responsible for obtaining accurate referral information and tracking daily

·       Obtains specific physician orders of both home care and hospice services

·       Ensures that physician’s orders are communicated to the agency’s staff with accuracy, thoroughness and understanding and follow up is made to the referring party

·       Communicates patient’s data and observations verbally and through recording to agency’s staff

·       Coordinates new patient’s care and any changes in patient care with director of nursing, case managers, physicians and with patients

·       Performs patient care responsibilities considering needs specific to the standard of care for patient’s age

·       Ability to adequately assess and reassess pain verbally over the phone.  Utilizes appropriate pain management techniques.  Educates the patient and family regarding pain management.

·       Performs support of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors

·       Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position

·       Follows the five medication rights and reduces the potential for medication errors

·       Addresses and supports cultural practices, as long as such practices do not harm others or interfere with the planned course of medical therapy

·       Participates in the agency’s information management and performance improvement programs

·       Complies with the agencies policies and procedures

·       Assists administration in meeting the Federal, State and location regulations and standards

·       Attends interdisciplinary team meetings

·       Follow up calls made and notated to discharged patients off home care 30, 60, 90 days out  and 6 months

·       Weekly coordination of HHA schedules (taking into consideration vacations/sick)

·       Completes annual education requirements

·       Maintains regulatory requirements

·       Maintains patient confidentiality at all times

·       Reports to work on time and as scheduled, completes work within designated time

·       Completes in-services and returns in a timely fashion

·       Attends annual review and department in-services as scheduled

·       Attends required staff meetings, reads and returns all monthly staff meeting minutes

·       Represents the organization in a positive and professional manner

·       Patient visits when needed

·       Take call on an as needed basis

·       Updates on call service  on necessary information and new admits daily

·       Actively participates in performance improvement and continues quality improvement (QI) activities

·       Complies with all organizational policies regarding ethical business practices

·       Communicates the mission, ethics and goals of the organization

·       Adheres to dress code, maintains neat and professional appearance, wears identification (if required)

·       Performs any other duties related to the position of in-take nurse/patient care services as directed by the administrator or director of nursing for both home care and hospice.

Working conditions:

Requires corrected vision and hearing to normal range.  Requires working under stressful conditions or working irregular hours at times.  Requires ability to transport self to and from work.
